How to Enroll
Children must be 3 or 4 years old before December 1 of the program year.
Families must present the following when registering their child:
- Official Birth Certificate
- Up-to-date Immunization Record
- Medical Insurance Card
- Proof of Income

If a child is eligible for the program, families will receive an application form, a Child Information Card, physical and dental forms to complete and return.
A letter of acceptance will be given to qualifying families, along with information regarding our Orientation Session and scheduling a Home Visit.
